Metode de tratament în bolile aortoiliace - experienţa clinicii de cardiologie Târgu Mureş

  • Subiect: A study was made to determine the success and patency rates with balloon angioplasty (PTA) and stent, associated with excimer laser angioplasty, in iliac arteries. Material and methods: One hundred sixtyone iliac interventions were performed in 116 patients with iliac or aortoiliac obstructive diseases by a single operator. Lesion type was TASC D- 29.41%, TASC C –20.59%, TASC B – 29.41% and TASC A- 20.59%. Laser angioplasty was associated in 14 cases(10 in iliac lesion only and 4 in aortoiliac lesion, and 88 iliac stents were implanted. Follow-up was performed twice a year. Results: Technical success was 98.27%. Doppler index increased with 51% in PTA group, 93% in stent group and 79% in laser group. Primary patency rates were 96.91% at 6 months, 93.06% at 1 year, 89.98% at 18 months and 88.34% at 2 years, and were superior in the PTA group (93.43%) than in the stented group (81.25%). Secondary patency rates were 95% respective 100%. Survival rate was 93% at 30 months in both groups. Conclusions: Iliac angioplasty appears to be safe and effective in iliac lesions. Indications were extended to more TASC C and D cases, with very good results regarding patency, survival and amputation-free rates during follow-up.
