Întoarcerea venoasă pulmonară total aberantă – elemente de diagnostic ecocardiografic

  • Subiect: As with other types of congenital heart disease (CHD), advents in cardiovascular surgery in newborns and infants, have heightened the importance of early recognition, accurate diagnosis, and better understanding of abnormalities of the pulmonary veins. Total anomalous pulmonary venous return (TAPVR) is a congenital heart defect in which there is no connection between the pulmonary veins and the left atrium, connecting instead directly to the right atrium or to one of the systemic veins by an alternate pathway. According to Darling’s anatomic classification, there are four types of TAPVR, based on the site of pulmonary venous drainage: type I – supracardiac, type II – intracardiac, type III – infradiafragmatic, type IV – mixed. Based on the pulmonary venous flow type, hemodynamically we distinguish another two forms: with or without pulmonary venous obstruction. Echocardiography is nowadays the most useful method in the diagnosis of TAPVC, but require knowledge about embryology and hemodynamic of pulmonary circulation. In this paper we reviewed the most important embryologic and echocardiographic issues in the diagnose of TAPVC.
