Corecţia defectului septal interatrial pe cord în activitate

  • Subiect: Objective: classically, the correction of ASD is performed using cardiopulmonary by-pass with aortic cross-clamping. The ischemic period has significant secondary structural and functional effects on myocardial tissue. The aim of the study was to perform the surgical correction of ASD on beating heart, without aortic cross-clamp. According to our data this is the first published study on the topic. Methods: between June 2004 to March 2007 we performed this procedure to 5 patients, 4 females and 1 man, with the average age of 39,8(limits at 10 to 58 years). We performed the closure of ASD in all the patients by suture in cardiopulmonary by-pass; in one case we performed partial tricuspidian anuloplasty (postero-septal) on Gore-Tex bandelet. Results: the average duration of surgery was 128 mins.(limits between 123 and 134), with an average of 19,5 min for cardiopulmonary bypass (limits between 16 and 22 min). A single patient with severe pulmonary hipertension and tricuspidian regurgitation needed inotropic support, medium dosage. The Intensive Care Unit stay was 1,2 days, and the average length of stay after surgery was of 8 days (imits between 4 and 11 days). No surgical complications occured. Conclusion: the correction of ASD on beating heart is a safe and riskless method.The mentioned technique is shortening the BCP and intervention time, protecting the myocardium against the adverse reactions of ischemia and reperfusion.
